I give permission for my child to attend the Six Flags Magic Mountain trip with First Baptist Church Teen Camp on Wednesday, July 8, 2026. I understand that transportation will be provided by the church, and I authorize the church leaders to obtain emergency medical treatment for my child if I cannot be reached. I understand that reasonable supervision will be provided throughout the trip.
